A gym admission form: what to include, with a template
In short
A gym admission form, also called a gym membership form, is the sheet a new member fills in on joining. It records who they are, their health, the plan they bought and what they agree to. A good one fits on one page and is the same for every member.
#What the form must do
It has four jobs. It identifies the member. It records a health declaration so you know about any condition before the first workout. It records the plan and payment so there is no dispute later. And it carries the member's agreement to your rules and policies.
Keep it to one page where you can. A long form is filled carelessly, and a carelessly filled form is not much use.
#The template
Section 1: Member details
| Field | Note |
|---|---|
| Full name | As on an ID |
| Date of birth | Age matters for safety |
| Gender | Optional |
| Mobile number | The number you will message |
| WhatsApp number | If different |
| Optional | |
| Address | For records |
| Emergency contact: name, relation, number | Not the member's own number |
| Photo | Only if you need it for the record or the access system |
Section 2: Health declaration
| Question | Answer |
|---|---|
| Any heart condition, high blood pressure or diabetes? | Yes / No |
| Any injury, surgery or joint problem in the past year? | Yes / No |
| Any asthma or breathing problem? | Yes / No |
| Currently pregnant or recently delivered? | Yes / No |
| Any medication that affects exercise? | Yes / No |
| Other things the trainer should know | Free text |
Add the line: "I confirm the above is correct and I will tell the gym if anything changes." Ask whether the member's doctor has cleared them for exercise if any answer is yes. Do not make medical judgments yourself.
Section 3: Goal and source
- Main goal: fat loss, muscle gain, fitness, rehab, sport.
- How did you hear about us? (Walk-in, Instagram, Google, referral, ad.)
The second question is worth the line. It tells you which door your members come through. See where gym leads come from in India.
Section 4: Plan and payment
| Field | Note |
|---|---|
| Plan name and duration | For example, quarterly |
| Start date and end date | Write both |
| Personal training pack, if any | Sessions and trainer |
| Amount paid, balance and due date | If a part payment |
| Payment method | Cash, UPI, card |
| Receipt or invoice number | For cross-checking |
Writing the end date on the form is the single best protection against an argument at renewal.
Section 5: Consents and agreement
- I have read the gym rules and agree to follow them. See the sample rules list.
- I accept the freeze, transfer and refund policy. See the freeze and transfer guide.
- I consent to receive membership messages on the numbers above.
- If you use biometrics: I agree to my fingerprint being used for entry, with the data handled as explained by the gym. Have this wording reviewed by counsel before you use it.
Keep marketing consent separate from service messages. A member who ticks one has not necessarily ticked the other.
Section 6: Signature
Member signature, date, and staff name. If a minor joins, add a parent or guardian signature.
#Handling the form
- Fill it at the desk, not at the door. A member rushing to a workout writes nonsense.
- Check for blanks. Mobile number and emergency contact are not optional.
- Store it. Paper in a locked cabinet, or a scan in a folder with restricted access.
- Only collect what you use. Every extra field is something you must protect.
- Give a copy of the rules. A photo on WhatsApp is enough.
#Mistakes to avoid
- A form that asks for an ID number you never use.
- No end date on the plan.
- Health questions with nowhere to write the answer.
- A form that exists only on the owner's laptop.
#Where Helmr fits
Helmr, gym management software for India, is being built so that the details on this form become the member's record once, with the plan, payments and check-in attached to it. It is in early access, and its price is per business plus an add-on per branch, published before launch.
Helmr is in early access for Indian gyms. If you would like to be among the first to use it, message us on WhatsApp.
Frequently asked questions
What should a gym membership form include?
Member details, an emergency contact, a health declaration, the plan with start and end dates, payment details, consent to the rules, and a signature.
Is a health declaration compulsory in a gym form?
It is good practice. It tells your trainers about conditions to be careful with. Ask a local lawyer how it fits your liability terms.
Should the form be paper or digital?
Either works. What matters is that every member fills the same fields and that the records are stored safely and can be found later.
Can a gym ask for an ID proof?
Some gyms do. Collect only what you will use, and store it safely, because anything you hold is yours to protect.
